This is a supreme and bad brains collaboration sweatshirt from the 2008 collection. You may be familiar with it if you were following supreme at the time. The bad brains and supreme collaboration items sold out instantly and were produced in low quantities. Available for sale only at authorized supreme retails stores, meant people queueing for days to purchase one of the items such as the sweatshirts, t shirt and vans sk8-hi shoes.

The condition of this is in a state called "dry rot", or "acid rot", which basically means that the dyes used to pigment broke down the strength of the cotton fiber, specifically the color black. 

This effect causes tears and the item is considered fragile or perhaps not suitable for heavy wearing

It is technically wearable, because it has some flexibility...but at the wearers risk!
